Electronic Signature & Communications Consent

By agreeing to this E-Sign Consent, you consent under the federal Electronic Signatures in Global and National Commerce Act (15 U.S.C. §7001 et seq.) and the Uniform Electronic Transactions Act to receive any and all disclosures, agreements, notices, statements, tax forms, and other communications (collectively, "Communications") from Initio Capital, Inc. and Column N.A. in electronic form.

Hardware and software requirements

• A device with internet access
• A current version of Chrome, Safari, Firefox, or Edge
• An active email account capable of receiving HTML email and PDF attachments
• Sufficient storage to save, or a printer to print, any Communications you wish to retain

Withdrawing consent

You may withdraw consent at any time by emailing support@initiotreasury.com. Withdrawal of consent will result in closure of your Account if paper delivery cannot be reasonably accommodated.

Requesting paper copies

You may request a paper copy of any Communication free of charge during the period it is required by law to be retained by writing to: Initio Capital, Inc., 548 Market St #95421, San Francisco, CA 94104.

Updating contact information

You agree to keep your email address current in your account profile. You are responsible for all Communications sent to the email address on file.
